Are you passionate about supporting learners with SEND to achieve their full potential, and successfully move into their next stage of education, employment, or independence and into adulthood? We are seeking an enthusiastic and committed Inclusion Coach - Transition Lead to join our dedicated INSPIRE SEND Support Team.

About the Role
This is an exciting opportunity to play a pivotal role in shaping and strengthening transition pathways for learners with Special Educational Needs and Disabilities (SEND) across the College.
As the Transition Lead, you will coordinate and deliver high-quality, inclusive transition support for learners entering the College, moving between programmes, and progressing to positive destinations beyond college. You will work collaboratively with curriculum teams, support services, and external stakeholders to ensure learners experience a smooth, planned, and successful transition. You will need to be able to drive and have access to a car

Key Responsibilities
Lead and coordinate effective transition arrangements for learners with SEND, including entry, internal progression, and exit routes
Design and deliver tailored transition programmes for learners with high needs or other vulnerable cohorts
Organise and oversee bespoke taster sessions and transition activities
Track and monitor learner progress and transition data to inform timely interventions
Work closely with the SENCO, SEND Team Leader, Inclusion Coaches, and academic teams to identify and support learners at risk of not progressing
Deliver targeted interventions (1:1 and small groups) to build independence, confidence, and readiness for next steps
Support EHCP processes, including annual reviews, support plans, and monitoring outcomes
Engage with parents, carers, and stakeholders to ensure clear communication and collaborative planning
Contribute to staff training and promote inclusive practice across the College

About You
We are looking for someone who:
Has a strong commitment to inclusion and improving outcomes for learners with SEND
Understands the principles of Preparing for Adulthood and enabling independence
Has experience supporting learners with additional needs in an educational setting
Is highly organised, proactive, and able to coordinate multiple transition activities
Demonstrates excellent communication, collaboration, and problem-solving skills
Can inspire confidence, resilience, and aspiration in learners
Is confident in working with EHCP processes and multi-agency partnerships
